How to renew by mail, fax or online

It's important to renew your CHIP/Children's Medicaid coverage on time.

Three months before your children's coverage is due to end, we will mail you a renewal packet. It will have an application with some of your information already filled in.

Online

If you have a username and password for this website, you can log on, fill out your CHIP renewal application, and submit it online.

If you do not have a username and password for this website, we will send you a temporary password and username you can use to log on then fill out and submit the application. If you have given us your e-mail address, we'll send this to you in an e-mail. Otherwise, we'll mail you the temporary password and username. You will get the password and username a few days after you get your CHIP renewal packet in the mail.

Once you have your own username and password or temporary ones we send you, follow the instructions below:

  1. Read these instructions, then click this link: Renewal application.
  2. Log in with your username and password - your own or the temporary username and password we send you.
  3. Answer the questions and update any information.
  4. Click “Submit” after you finish the renewal application.
  5. You can log in and check your application status 48 hours after you submit your application.

Mail or fax

If you'd like to mail or fax in your renewal application, follow these steps:

  1. Look at the application we sent you in the mail and update your information.
  2. Fill in all the questions that are blank.
  3. Send us copies of at least one paycheck stub or other proof showing each family member's income and expenses.
  4. Sign and date the application. Send it to us in the pre-addressed envelope or by fax.

Enrollment Fee to Start/Renew CHIP Coverage

If you need to send an enrollment fee to start or renew your children's CHIP coverage, please send your check or money order to the following address:
