How to renew by mail, fax or online

It's important to renew your CHIP/Children's Medicaid coverage on time.

Three months before your children's coverage is due to end, we will mail you a renewal packet. It will have an application with some of your information already filled in.

Mail or fax

  1. Update your information on the application.
  2. Fill in all the questions that are blank.
  3. Send us copies of at least one paycheck stub or other proof showing each family member's income and expenses.
  4. Sign and date the application. Send it to us in the pre-addressed envelope or by fax.

Online

If we sent you a CHIP renewal packet and a temporary password you can fill out an online renewal application and send it to us through this website.

  1. Read these instructions then, click this link: Renewal application.
  2. Log in using your CHIP case number and temporary password.
  3. Answer the questions and update any information.
  4. Click “Submit” after you finish the renewal application.

Enrollment Fee to Start/Renew CHIP Coverage

If you need to send an enrollment fee to start or renew your children's CHIP coverage, please send your check or money order to the following address:
