How to Apply
Apply Online
You can fill out and submit an application anytime of the day or night. If you don't have a username and password for this site, you will need to create these before you fill out the application. You will also need to fax or mail us your documents of proof such as paycheck stubs and proof of expenses.

Call us toll-free Monday to Friday from 8 a.m. to 8 p.m. Central Time and we'll take your information over the phone. Then, we’ll mail your filled out application to you for you to sign.
Next, get your documents of proof together (paycheck stubs, proof of expenses, etc.). Mail them to us, along with your signed application using the postage-paid envelope that comes with your application.
You also can fax the signed application and documents to us at 1-877-542-5951.
Apply by Mail or Fax
We will send you an English/Spanish application, instructions for filling it out, and a postage-paid pre-addressed envelope. Fill out the application, get together your documents of proof (paycheck stubs, proof of expenses, etc.), and mail or fax the application and documents to us. If you have questions, call us toll-free 1-877-543-7669 Monday to Friday, 8 a.m. to 8 p.m. Central Time.
You also can print an application here or pick up a copy of the application at H-E-B pharmacies.
Important Tips:
- Make sure you answer all the questions on the application. If the question doesn’t apply to your household, write “N/A” next to the question.
- Sign your application.
- If you have questions about the documents of proof you need to send us, call us toll-free 1-877-543-7669 (1-877-KIDS-NOW).
- If you are applying for CHIP perinatal services fill out the Report of Pregnancy form, have your doctor sign it, and send it to us along with your completed and signed CHIP/Children’s Medicaid application.
- You can log in and check your application status 48 hours after you submit your application.
