After You Apply

We have up to 45 days to work on your application. When we finish, we will send you a letter to let you know if your children are approved for CHIP, or Medicaid, or neither program. If they are approved, we will send you a packet telling you how to pick a medical and dental plan, and how to pick a main doctor and dentist. CHIP coverage cannot begin until you pick a medical and dental plan, sign and return the Enrollment/Transfer Forms, and send in your enrollment fee if you owe one.

Filling Out the Enrollment/Transfer Forms

  1. Pick a medical plan and a dental plan.
  2. Sign both the Enrollment/Transfer Forms.
  3. Return the Enrollment/Transfer Forms by the due date.
  4. Pay the enrollment fee by the due date, if there is a fee.*

*If you have an enrollment fee, your packet will include a payment coupon. Families can pay the CHIP enrollment fee by credit card online or or by sending a check to:
